Our loving husband, devoted father, beloved son, brother, uncle, cousin and friend passed away Wednesday, Aug. 7, 2013, in Salt Lake City, Utah.
Chad married the love of his life, Shanna Jo Ertmann, on May 5, 2000. He was an amazingly compassionate and loving father to his daughter Madelyn and son Gaven. He was a talented artist and passed down that talent to Madelyn. He was an avid Real Salt Lake fan, and loved all things soccer.
Chad is survived by his wife, Shanna, daughter Madelyn Olivia, son Gaven EuGene, mother Marlene Ellis, siblings Candy (Monte) Symmonds, Steven (Diane) Ellis, Holly (Jerry) Hyatt, Tammy (David) Tibbitts, Michael (Maria) Ellis, April (Eric) McMurtrey, and Brandon (Kim) Ellis.
He was preceded in death by his father EuGene Leon Ellis and grandmother Norene Calton Smith.
Funeral services will be held Tuesday, Aug. 13 at 11 a.m. Visitation will be held Monday, Aug. 12 from 6 to 8 p.m., and 10 a.m. Tuesday, Aug. 13 at the LDS Meetinghouse on 2700 W 7000 South in West Jordan, Utah.
In lieu of flowers, a fund has been set up for his children at Cyprus Credit Union under Chad Ellis' Kids Donation Account.
