Appointment of Personal Representative through a Written Directive
If your loved one has established a written directive that has been notarized or executed with the same formalities required for a will under Section 75-2-502, kindly provide a copy to our staff when arranging the necessary services. It is essential to note that, in accordance with the law, the funeral home is mandated to coordinate arrangements with the designated personal representative or the legal next-of-kin.
Please be aware that Utah Advanced Health Care Directives and Powers of Attorney cease to be valid upon the individual's passing.
Attire
We kindly request that you bring all clothing items you wish your loved one to wear during their memorial services. This attire may include any suitable clothing, provided it is long-sleeved and has a high neckline. Please remember to include undergarments, socks, a belt, shoes, jewelry, and any other accessories. We appreciate it if you could bring these clothing items in a bag or container that can be returned at a later time.
Utilizing a Prepaid Funeral Plan for Payment
Prepaid funeral plans arranged for and through Affordable Funerals and Cremations are administered by Security National Life Insurance Company, with our funeral home designated as the beneficiary. These plans typically encompass various services and items, including funeral services, viewings, the use of funeral vehicles, transportation of your loved one, preparation of your loved one, the casket, and the vault. The specifics of the prepaid plan determine which items are guaranteed and paid for. Since these items have been prepaid, Security National Life Insurance Company disburses the funds to the funeral home accordingly.
Please be aware that certain items, such as sales tax, death certificates, flowers, programs, and similar expenses, cannot be prepaid. Consequently, these costs are the responsibility of the family and must be settled at the time of need.
We are pleased to accept prepaid funeral plan policies from other funeral homes and insurance companies. We will provide services and items as listed in the policy paperwork, and the funding process mirrors that of Security National Life Insurance Company.
Using a Life Insurance Policy to Cover Costs
Life insurance policies provide funds rather than guaranteeing specific services or merchandise. There are two potential methods for utilizing a life insurance policy:
- Beneficiary Claim: You, as the policy beneficiary, may submit a claim to the insurance company to access the policy funds. Please note that this process can take approximately 30 to 90 business days to complete. We do require full payment 48 hours prior to the scheduled services, cremation, or burial. Therefore, it is possible that your family may need to cover the expenses initially, with the insurance funds reimbursing you upon receipt.
- C&J Financial: We also offer the option of working with C&J Financial, a company that facilitates the use of insurance policies. C&J contacts the insurance company on behalf of the beneficiary to verify the policy's assignability and contestability status. They also confirm the amount available from the policy. This verification process typically takes about 3 to 5 business days. Upon verification, C&J provides upfront funds for services while managing the policy and claim paperwork in the background. It's important to note that, to proceed with this option, C&J charges a 5% fee based on the total balance.
If you opt for the C&J Financial option, we will not schedule services or order merchandise until we receive verification from C&J. Once we have received confirmation, we proceed accordingly. However, if we do not receive verification 48 hours prior to the funeral services, we may either A. cancel or postpone the services, or B. request that the family covers the expenses upfront. Subsequently, we will refund the family the amount they paid once we receive funds from the insurance company.
To utilize C&J Financial, we will require the following information: the name of the insurance company, insurance policy number, beneficiary name, address, phone number, email address, social security number, a contact person in the company's claims department, and the benefit amount of the policy.
